our Assisted Living Program.
WHAT IS AN ASSISTED LIVING PROGRAM?
An Assisted Living Program is licensed by the Department of Health to provide health
related services and personal care at home.

ELIGIBILITY
Care Strategies is available to residents residing at a Heritage Village Community who require assistance with three or more activities of daily living:

Locomotion-includes both indoor outdoor mobility

Bathing
-
Dressing upper and/or lower body
-
Transfer toilet and/or toilet use assistance
-
Bed Mobility
-
Eating
-
Transfers (how you move from one surface to another. For example, getting up from a chair)

HOW DO I QUALIFY?
If you are a resident residing at a Heritage Village Community who needs a level of care, which means requiring assistance with three or more activities of daily living, such as bathing, dressing and locomotion, you qualify for Care Strategies. HOW CAN I AFFORD THE ASSISTED LIVING PROGRAM?
You may qualify for our Assisted Living Program through the Medicaid Managed Long Term Care Services and Support benefit. Affordable private pay options are also available. Contact Care Strategies for your free consultation.
DO I HAVE TO PARTICIPATE IN CARE STRATEGIES IF I LIVE AT A HERITAGE VILLAGE COMMUNITY?
No. The Assisted Living Program is a voluntary program.
DO I HAVE TO GO OUTSIDE OF MY BUILDING TO RECEIVE SERVICES?
No. Our Assisted Living Program provides services within your Heritage Village Community.

CAN I CONTINUE TO SEE MY PRIMARY PHYSICIAN?
You may continue to receive care under your primary care physician. Care Strategies Assisted Living Program can also coordinate a visiting physician on site.
HOW LONG CAN I BE ON THE PROGRAM?
Your eligibility is determined by assessing your needs. Those residents using Medicaid benefits will receive periodic reassessment by Medicaid case managers.
